How to use Diet ID with Cerbo EHR

How to get started

1. If you are already a Cerbo customer and a Diet ID customer, tell your Diet ID account manager that you are a Cerbo customer and would like Diet ID data posted in real time to the patient chart in Cerbo

2. Within 24-48 hours, your custom Diet ID app will be updated to send Diet ID results directly into the Cerbo patient chart. 

How to get patients to take the assessment

We recommend asking patients to take the assessment as a part of intake and then repeatedly throughout the year as a way to measure progress. 

Options: 

1. Add your custom Diet ID link (customname.dietid.com) to an onboarding message template with the following message

Please complete this 2-minute image-based dietary assessment to evaluate how healthy you eat today and how you can improve. Your provider will review the results with you at your next visit. Click here and register for a new account with your Cerbo email address (customname.dietid.com)

2. Include signage for in-person visits like the following

Please ask your account manager for a custom-branded sign for in person visits

You be sure to include a repeat assessment during follow up visits. The assessment request can be done before the visit or with the MA during the visit. 

How you will receive Diet ID's results in your patient's electronic health record

Once a patient completes the Diet ID assessment, the results will show in the "Questionnaires" section of the patient chart. If you would also like an email notification every time a patient completes an assessment, please ask your account manager to enable notifications. 

This is how you'll see notification of a completed Diet ID assessment within a Cerbo patient chart. Click on Save Document and Review. 

You'll see this report, which includes the Diet Quality Score (1-10) and a link to view the assessment report. Click on the link next to "Your assessment" to view the report.
