To add Diet ID to your Healthie account, fill out the form here: https://dietid.com/gethealthie.

As a Healthie provider, you can access Diet ID's digital dietary assessment at a discounted price of $9.99 per month. 

Once we receive your request:

• You'll receive an email including the step by step instructions of what happens next
• You'll receive an agreement to sign, which covers data and privacy
• You'll receive payment instructions
• We'll enable the Diet ID assessment from within the Healthie platform so you can add that custom form to any intake flow.

Most clients will add the Diet ID assessment to their standard intake process or nutrition visit workflow so they can get an instant dietary analysis each time they see a patient.
