Healthie: How does my patient fill out the assessment?

If you want your patient to fill out the Diet ID assessment, be sure to share the custom form from your Healthie account with your patient. You can share the form as a one-off, or you can embed it into your intake workflow (recommended). 

1. Click on Forms from the left navigation > Intake Flows > Select your intake flow > Add form to Intake Flow > Search for "Diet ID Assessment" > Check of Set form as required > Click Add 

Click Add Form to Intake Flow on the confirmation screen. 

Once your patient receives the form, they will be prompted to click on a link to complete the Diet ID assessment. 

The assessment can be accessed on any web-enabled device. Your patient will be prompted to register with Diet ID. They can use their email address and set up a new password, but at this time, they will NOT be able to log into Diet ID with their Healthie username and password.


Once your patient signs up, they will complete a short, but comprehensive dietary assessment using images. 

It starts with a few questions about what food groups you eat

Then you will be prompted to choose which image looks more like how you typically eat. You'll choose anywhere from 5 - 10 times. Once you've answered a few other personal questions, we'll generate a comprehensive dietary assessment. 

We also help people establish a goal diet by identifying health concerns they have.

Based on the selected goals, we identify styles of eating that are best and as you to choose your top pick. We have a comprehensive library of styles incluidng various therapeutic diets such as the Ornish Diet, Portfolio Diet, DASH diet, as well as ethnic variants such a Mexican style and South Asian style. 

The final assessment evaluates dietary style, quality level (using the Healthy Eating Index scoring system), nutrient intake, food group intake, nutrient gaps, food group targets, and more.

Results are generated in real time. Once your patient completes the assessment, they are prompted to add the link to their report to their Healthie intake form. They simply click COPY TO CLIPBOARD... 

And paste the link in the healthie form 

You'll receive the results in the form a link submitted via the Diet ID assessment form via Healthie. 

View the patient's completed form by clicking on FORMS > VIEW
